If loved one is still in capable decision making
(Lawyer or Not for some forms and states- I did mine on eForm.com for a $49 a month script I cancelled before 30 days)
Durable POA- with Financial Decisions and provide to all Dr.s and your bank
Medical POA- Provide to All Dr's
Advanced Directive for Medical (provide to all Dr's)
Your bank can sign these as a Notary, you will need two witnesses.
Will (Lawyer will be needed)
Compassionate Care Review and Last Wishes
Make sure you have Authorization to Release Medical Records at all Dr's offices signed while capable of decision making, and provide the Dr. with your POA copy, changing phone on pharmacy and dr to you.
SCAN all documents in and store on a Thumb Drive (make a a few copies and have hard copies to share in a file on visits.
I also have been making videos of tough conversations in case other family members decide later on they want to step in. I want my spouses wishes clearly appreciated and not mis understood.
Print out and go through the stages with your partner, and make notes on how they feel about each one and what their wishes are for care at that point. (good time to video)
Remember Assistance, Memory Care, Respite Care, Home Health, Hospice are all items you need to plan for mentally and financially.
Check through health insurance policy and see what is offered for in home health etc.
